Registering to use the patient
portal will be available to you after you have completed your initial
appointment at OA. You must sign-up prior to logging in. Please note that your
electronic medical record is in draft form for 48 hours after your appointment.

Usage Information
1.
Once
you have completed registering for access to the patient portal:
a.
The
patient portal is available 24/7 for accessing your medical record, and
reviewing your medical summary, medical history, and medications list.
b.
You
may also update your personal information, medical history, and medications
list within 7 days prior to an active appointment.
2.
Create
your login and password to access the patient portal by clicking the
“sign up” button in the “Login to Patient Portal”
section to the right. To sign up
successfully, OA must have your active e-mail address on file in your patient
record. This email address must be specific to you, and not included in any
other OA patients record. Once you
have signed up, you will receive a confirmation email from Phoenix Ortho with a
link to activate your account.
